Elementary Community Registration Instructions

1.     JeffCo Connect—Create an account or update existing account here.  Additional instructions can be found here.

2.     Elementary Registration Packet—Please complete each form and submit either via hard copy or e-mail to

3.     Submit immunization records (from your pediatrician) Families requiring an exemption can find the form here.

4.     Community Handbook—Please review for familiarity with our policies and procedures, then sign and return the agreement at the end of the registration packet.

5.     For students using Extended Care—Extended Care Registration Packet & ATS Setup (You should receive an email with subject “Please set your CDHS Child Care Attendance System PIN” from CDHS Attendance Tracking.)

6.      Families requesting reduced or waived tuition or fees, please complete and submit the    Family Economic Data Survey

7.     Required as needed for individual student: 
        Colorado School Asthma Plan
        Colorado Allergy and Anaphylaxis Emergency Care Plan
        Medication Administration Plan
        Cream/Ointment/Balm Permission Form

8.     Please have the following forms transferred to GWMS if they apply to your family:
        IFSP or IEP for students with identified special needs
        Student Health Plan or 504 Plan
        Significant Observations of the child's development
        Record of parent conferences, previous school records, etc.
